PHYSICAL EXAMS

Physical exams are required annually for all students playing a sport or taking PE.  Physical exams will be conducted on Wednesday, June 9th, 2010 from 2:45 until 5:00pm.  No appointment is necessary - first come, first served.  All who arrive by 5:00 will receive a physical exam.

The cost is $15; wear t-shirts, shorts, and athletic shoes. Our doctors will complete the Physical Exam Form during your the exam; complete the rest of the medical forms ahead of time and bring them with you to the exams.



If you choose to have your doctor conduct your physical, please plan ahead:  make the appointment soon (summer is often a difficult time to get an appointment), take the Physical Exam Form with you to the appointment so your doctor can complete it, and YOU return it to Bishop Blanchet by July 31st.
