SPORTS PHYSICALS UPDATE FOR 2020-2021 ATHLETIC PARTICIPANTS Bishop Blanchet strongly encourages all athletes to get annual physicals. Given the ongoing Covid-19 pandemic and the challenge of scheduling appointments with family doctors, Bishop Blanchet will accept physicals dated within the last two years for the 2020-2021 school year. Note: physicals cannot expire during a season an athlete is participating in. If a physical is set to expire before or during a season, the athlete must have a new one before they can try-out or participate in the remainder of that season. All athletes will need to submit an updated medical history form for the 2020-2021 school year. All documents will be submitted via FamilyID. FamilyID registration will open on July 6. Please email Athletic Director Sean Gaskill (firstname.lastname@example.org) with any questions.
In order to participate in a sport, all student-athletes must register through FamilyID.com. FamilyID.com is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. FamilyID allows you to create a profile for each of your students and then use that profile to register for different programs (Athletics) and events (Campus Ministry).
The exception to our online format is the pre-participation-medical history form and physician evaluation form. Both of these forms need to be uploaded through your FamilyID account. The medical history form will be filled out and signed by the student-athletes guardian and the physical form will be completed and signed by your health provider. Bishop Blanchet requires physicals to be renewed each school year. Click here for medical forms.
All athletes need to complete their online forms, including uploading a current physical and medical history form, to be eligible. Once forms are submitted online, the parent/guardian account holder will receive a confirmation email from FamilyID stating that their registration has been successfully submitted. The Athletic Office will then review the registration. Accounts will be sent an email stating that the student-athlete is eligible to participate or not eligible due to a specific reason.
The Athletic Office will be open starting on Monday, August 17 from 9 a.m.- 2 p.m. through Friday, September 4th for help with the registration process. Fall cut-sports consist of boys and girls golf, girls soccer, and volleyball. Fall non-cut sports are boys and girls cross-country, football, and girls swimming. Please check bishopblanchet.org/athletics for tryout information.
