PE Waiver Request Form

Blanchet students who play a BBHS sport or participate in a sport/dance program outside of school, are eligible to waive up to 1 credit of their required PE credits during their time at Bishop Blanchet.

If you would like to waive a PE credit please make sure to follow the instructions below.

If you are playing a Bishop Blanchet School Sport:

• Fill out the PE Credit Waiver Sign Up form completely. You will need to use your BBHS student email in order to access the form. You may also stop by the Academics Office to sign up using our kiosk.

• Once your sport is completed make sure to fill out the Application for PE Credit Exception form. This must be signed by you, your parent/guardian, and your coach.

If you are participating in a sport/dance program outside of Bishop Blanchet:

• Fill out the PE Credit Waiver Sign Up form completely. You will need to use your BBHS student email in order to access the form. You may also stop by the Academics Office (Room 115) to sign up using our kiosk.

• You must also fill out the Proposal Form and turn it into the Registrar (in person or via email) before your sport begins. You can also pick up a printed copy of the proposal form in the Academics Office.

• While your sport is ongoing you will need to keep track of your hours by filling out the Activity Log.

• Once your sport is completed make sure to fill out the Application for PE Credit Exception form. This must be signed by you, your parent/guardian, and your coach. You will need to turn in the exception form along with your log to receive credit.

Please make sure to complete all steps of the credit waiver process or you may not receive credit.
