Medication and Health

As we prepare for the upcoming school year, we want to ensure that we have the most accurate and up-to-date information regarding your student’s medical needs. If your student has a medical condition that requires support during the school day, please take a moment to review the information on this page and complete any necessary forms. Once completed, please return to Lynn Augustavo, Associate Director of Student Services.

Applicable forms must be completed before the start of every school year.

Prescription Medication Form
This is a general form for any prescribed medication, including those for ADHD, asthma, and other medical conditions. It must be completed by a licensed healthcare provider and signed by a parent or guardian. This must be completed before the start of every school year.

Severe Allergy
Form

This form is for an Epinephrine Auto injector (EpiPen, AuviQ, etc.) and must be completed by a licensed healthcare provider and signed by a parent or guardian. This must be completed before the start of every school year.

Over the Counter MedicinE form 
This form allows specific BBHS faculty and staff members authorization to administer over the counter medication to students. This must be completed before the start of every school year.

Seizure
This form is for students with a seizure diagnosis who have been prescribed an emergency anticonvulsant (e.g., Diastat, Midazolam). It must be completed by a licensed healthcare provider and signed by a parent or guardian. A seizure care plan may also be required. This must be completed before the start of every school year.

Diabetes
Ask your student’s endocrinologist to provide a School Health Plan. Even if your student manages their diabetes independently, a School Health Plan is still required. This must be completed before the start of every school year.
